Care Plus NJ, Inc. (“CarePlus”) a 501(c)(3) non-profit organization, provides comprehensive,

recovery-focused integrated primary and mental health care and substance abuse rehabilitation services.

Our outpatient services are offered on a voluntary outpatient basis to adults and children at a total of

24 sites, including four outpatient centers (located in Paramus, Fair Lawn, Hasbrouck Heights,

and Montclair), ten residential facilities, offices at three local hospitals, and seven community offices,

all conveniently located throughout Northern New Jersey.

Family Support Organization of Bergen County
The New Jersey Department of Children and Families, Division of Children’s System of Care serves

children and adolescents with emotional and behavioral health care challenges and their families. CSOC

is committed to providing these services based on the needs of the child and family in a family-centered,

community-based environment.

The Center for Alcohol and Drug Resources

The Center for Alcohol and Drug Resources, a Program of Children’s Aid and Family Services, provides

substance abuse prevention, education, information, and referral services to county residents, businesses,

schools, faith-based organizations, municipal alliances and social service agencies. The Center is supported

by grants, contracts and generous contributions from public and private donors. As a result, many of our

services are offered to the public at no cost. The Center for Alcohol and Drug Resources is The Prevention

Resource Center for Bergen County and towns East of the Passaic River in Passaic County. We are located

in central Hackensack in a contemporary and inviting setting with a knowledgeable staff. Our office is open

Monday through Friday from 8:00 a.m. until 5:00 p.m. Our services are professional, confidential and

affordable. We’re here to help you.

Bergen’s Promise 
Bergen's Promise is the designated Care Management Organization (CMO) for Bergen County, New Jersey.

We serve youth ages 5 to 21, with serious behavioral health challenges, substance abuse issues and

developmental disabilities. Youth with chronic co-occurring behavioral and specific medical health issues

will also be served through an enhanced medical care coordination component. The goal of this additional

support is the joint coordination of wellness promotion with behavioral health care management. 

The CAPE Center 
The mission of the Bergen County Special Services School District (BCSS) Resource Center is to serve

individuals with disabilities from infancy through adulthood as a central point of contact for guidance and

connection to accessible and appropriate resources. The Center is committed to helping individuals and

families access the system of resources to enrich all aspects of life.

The Arc of Bergen and Passaic
The Arc of Bergen and Passaic serves over 1,000 individuals with developmental disabilities and their

families. From Early Intervention for infants and toddlers to Summer Camp and After School Programs

for youth to Vocational Training, Job Placement, Adult Day Programs, Recreation, Group Homes and

Apartments for adults; The Arc is creating opportunities for individuals to reach their full potential and live

happy, productive and fulfilling lives.

Division of Family Guidance

Bergen County's Division of Family Guidance provides clinical, residential, educational, correctional, case

management, and monitoring services to at-risk children, adolescents, and families, particularly those who

are otherwise unable to receive needed services. It is Bergen County's resource for youth and families

in need.

Juvenile Family Crisis Intervention Unit (JFCIU):

Adolescent and Family Treatment (AFT):

an outpatient therapy program which offers consultation, counseling, and therapy services to youths

and families without insurance or access to services

Children's After-School Treatment Services (CATS):

an activity-based, therapeutic after-school program designed to help improve the social skills of elementary and middle school-aged children 

Multi-Systemic Therapy (MST): a federal "blueprint" program that addresses the needs of juveniles

with severe behavioral problems by providing intensive in-home family therapy

The Statewide Parent Advocacy Network (SPAN)
The Statewide Parent Advocacy Network (SPAN) is an independent 501(c)3 organization committed

to empowering families as advocates and partners in improving education and health outcomes for infants,

toddlers, children and youth. SPAN is a "one stop" for New Jersey families. SPAN is New Jersey's Parent

Training and Information Center; Family to Family Health Information Center; Family Voices State

Affiliate Organization; Parent to Parent USA affiliate; and a chapter of the Federation of Families for

Children's Mental Health.

The Bergen County Special Services CAPE Resource Center
The Bergen County Special Services CAPE Resource Center connects individuals with disabilities and

their families, as well as professionals in the field of disabilities, to accessible and appropriate services,

information, education, and resources. They are your one stop center for information about summer

camps/programs for children and adults with special needs. They offer workshops for parents and

professionals on a range of topics including transition planning, positive behavior supports, mental

health issues, guardianship, special needs trusts and estate planning, post-secondary educational options,

Department of Human Services Division of Disability Services NJ Resource Guide 2015

DDS has successfully provided this consumer friendly, useful tool, which identifies all levels of

government, community organizations and professionals working to assist people with disabilities.

The publication provides access to up-to-date information that individuals with disabilities, along

with their families, may need to flourish in their physical, professional and recreational lives. As

in previous years, this publication will be made available online in English, Spanish, and in audio

format, so that individuals and agencies can access just the services they need and can make copies
