If you are new to the Borough of Wood-Ridge and you wish to transfer your child into the Intermediate School (Grades 5 and 6 only), you can find everything you need on this page. If you have additional questions, please email Administrative Assistant, Ms. Gregory Email Ms. Danila Gregory. 

The following registration forms must be completed:  

• NJ Smart Registration Form

• Health - Emergency Form

• Home Language Survey

• Spanish Home Language Survey

• Physical Form (Immunization Records must be attached to this Physical Form)

• Medical Letter to Parents

 In addition, the following documents must be submitted:

• Copy of Original Birth Certificate

  • Transfer Card from Previous School (if applicable)

  • Proof of Residency:

    • if you own property- either a copy of deed, mortgage statement, or tax bill with a utility bill and driver's license

    • if you rent- a copy of the fully signed lease agreement, with a utility bill, and driver's license
